Key date reminder: Wednesday, July 14

July 13, 2021

Topic: Updates

As a reminder, tomorrow, Wednesday, July 14, is a key date for our middle school families.

First, this date marks five weeks before the first day of school. In order for eligible students (age 12+) to be fully vaccinated by the first day of school (Wednesday, August 18), they must receive their first dose of the two-dose Pfizer vaccine by Wednesday, July 14.

Receiving a vaccine is safe, free, and protects against severe illness. We strongly encourage CCSD21 families to vaccinate all eligible members of their household against COVID-19.  Make your appointment today! If you or someone you know needs assistance scheduling a vaccine appointment, call the toll-free Illinois Department of Public Health hotline at 833-621-1284. To learn more about the vaccine in young people, visit the Illinois Department of Public Health’s FAQ.

Second, Wednesday, July 14, is our Card Collection Day at CCSD21 middle schools. In order for CCSD21 to understand vaccination levels within our middle schools, we’re asking families to consider submitting a copy of their child’s completed vaccine card:

  • Email a scanned copy or photo of the vaccine card to [email protected].
  • Visit any middle school during our Card Collection Day on Wednesday, July 14:
    • Cooper: 8:30 – 10:30 AM
    • London: 11:00 AM – 1:00 PM
    • Holmes: 1:30 – 3:30 PM
  • Visit any CCSD21 school on or after Monday, July 26, between 9:00 AM – 3:30 PM.
